Win/Loss & W2G Requests
To request Win/Loss or W2G statements, please complete the following steps:
- Complete the request form located here or stop by our Credit office during normal operating hours to obtain a copy
- If you do not circle either Win/Loss or W2G's, we will assume you are requesting a Win/Loss
- Please note on the form if you will be picking it up or if you would like it mailed to your house
- Email the completed form and a scanned copy of your ID with a current home address to our Credit office at [email protected] OR drop off your completed form during regular office hours to our Credit office, outside of these hours forms may be dropped off to the Cage
- All form submissions must be accompanied by a photocopy of ID with the guest's current address
- Once received, we will process the requests and they will be sent to the guests via standard mail only. Please note, the W/L statements will not be e mailed.
- If you are requesting copies of your W2G's, it will take several days to process as the Revenue Audit department must retrieve those
